Parent’s Acknowledgement and Participation Release form I further agree for, my child and myself to indemnify and hold Michelle Sawka/MJB Performance employee from any and all claims or causes of action, including Lawsuits, for personal injuries or damages, including death, including but not limited to judgments and attorney’s fees, caused by or related to my daughter or son’s participation in MJB Performance events. This acknowledgement and release binds myself, my daughter or son’s other parent or guardian (if applicable), and daughter or son, and my successors or assigns.
